Old­er Adult Services

Ken­neth Young Cen­ter pro­vides com­pre­hen­sive in-home assess­ments and ser­vices to old­er adults in Elk Grove and Schaum­burg town­ships. We also pro­vide infor­ma­tion, assis­tance, and refer­rals to adults with disabilities. 

We help old­er adults and care­givers deter­mine their spe­cif­ic needs and the ser­vices avail­able to meet them. Our goal is to con­nect old­er adults to ser­vices that allow them to con­tin­ue to live as inde­pen­dent­ly as possible.

We are proud to be a SAGE­Care Cre­den­tialed Agency and com­mit­ted to sup­port­ing all adults as they age in our community.

Sup­port Groups and Edu­ca­tion­al Programs

Ken­neth Young Cen­ter’s Old­er Adult Ser­vices Depart­ment is com­mit­ted to pro­vid­ing ample resources to help old­er adults, care­tak­ers, fam­i­lies, and loved ones through dif­fi­cult and con­fus­ing transitions. 

Our Care­giv­er Resource Cen­ter pro­vides infor­ma­tion, edu­ca­tion, and resources to the fam­i­ly care­givers. A fam­i­ly care­giv­er encoun­ters a vari­ety of issues includ­ing health con­di­tions, changes in behav­ior, and rela­tion­ship issues, in addi­tion to the care­giver’s emo­tion­al response to these chal­lenges. The care­giv­er pro­gram offers a wide array of ser­vices and resources to empow­er the care­giv­er in cop­ing with their spe­cif­ic situation.

Edu­ca­tion­al pro­grams help fam­i­lies under­stand and learn cop­ing skills to man­age con­di­tions like Alzheimer’s, demen­tia, Parkinson’s, and more.

The process starts with a care­giv­er assess­ment and includes indi­vid­ual coun­sel­ing, edu­ca­tion­al pro­grams, and a care­giv­er sup­port group. A plan of care indi­cates the needs of the fam­i­ly and encour­ages plan­ning for the future. The Care­giv­er Spe­cial­ist sup­ports the fam­i­ly every step of the way as the sit­u­a­tion changes and offers ways to pro­mote self-care.

Rel­a­tives Rais­ing Chil­dren Sup­port Group

Senior rel­a­tives rais­ing chil­dren have a unique need for sup­port. In this group, grand­par­ents or fam­i­ly mem­bers give and get sup­port from oth­ers with sim­i­lar expe­ri­ences, share ideas and expe­ri­ences about rais­ing chil­dren, and get infor­ma­tion about resources avail­able to them.

This sup­port group meets the first Mon­day of every month from 10:00 a.m. to 11:30 a.m. at Ken­neth Young Cen­ter’s Elk Grove Vil­lage office.

Care­giv­er Sup­port Group

Car­ing for a loved one who has phys­i­cal chal­lenges, demen­tia, or a chron­ic ill­ness can be a dif­fi­cult job. Join oth­ers who are going through a sim­i­lar expe­ri­ence. In this care­giv­er sup­port group, you will meet oth­er care­givers, exchange ideas, learn about avail­able resources, receive sup­port, and feel empowered.

This sup­port group meets in-per­son. This group is held on the third Thurs­day of every month from 6:00 p.m. to 7:30 p.m. at Ken­neth Young Cen­ter’s Elk Grove Vil­lage office.

Mem­o­ry Café

The Mem­o­ry Café is a free month­ly event for peo­ple liv­ing with mem­o­ry impair­ments and their care­givers to attend togeth­er. Our Café Lead­ers cre­ate a wel­com­ing envi­ron­ment in which care­givers and peo­ple with mem­o­ry impair­ments can con­nect and share resources or sug­ges­tions with one anoth­er while engag­ing in activ­i­ties that pro­mote healthy inter­ac­tion. The Café is held on the third Fri­day of each month.

Infor­ma­tion / Assistance

Seniors or care­givers call for help, and we pro­vide infor­ma­tion and con­nec­tions to com­mu­ni­ty ser­vices. If an in-home assess­ment is need­ed, our intake process is initiated.

In-Home Assess­ment

A care coor­di­na­tor vis­its the home of the senior to con­duct a com­pre­hen­sive assess­ment of needs.

Care Coor­di­na­tion

With the input of the senior a care plan is devel­oped. The case man­ag­er con­tin­ues to mon­i­tor as needs change. Care coor­di­na­tors coor­di­nate com­mu­ni­ty based services. 

For seniors who meet income and dis­abil­i­ty guide­lines, case man­agers can engage the Illi­nois Depart­ment on Aging Com­mu­ni­ty Care Pro­gram (CCP), which includes home­mak­er ser­vices for per­son­al care, shop­ping, meal prepa­ra­tion and house­hold assis­tance. These inten­sive in-home ser­vices are meant to pre­vent pre­ma­ture nurs­ing home place­ment. CCP also pro­vides adult care, which offers a safe, struc­tured reha­bil­i­ta­tive envi­ron­ment and pro­vides care­giv­er respite.

Assis­tance in pro­vid­ing eli­gi­bil­i­ty screen­ing and appli­ca­tions for pub­lic care benefits.

Spe­cial Services

Choic­es

When frail elders and their fam­i­lies are con­sid­er­ing nurs­ing home place­ment, KYC staff screens appli­cants for eli­gi­bil­i­ty and assess­es for com­mu­ni­ty based ser­vices, nurs­ing home place­ment, or oth­er long term options. KYC staff also com­pletes screens in area hospitals.

Noon­time meals are deliv­ered on week­days to those who are unable to man­age prepar­ing their own and are homebound.

Offers free health insur­ance coun­sel­ing for Medicare ben­e­fi­cia­ries and their caregivers

Care­giv­er Resources

Edu­ca­tion on resources and skills for the care of an elder­ly fam­i­ly mem­ber or friend. Tips for self-care and respite, peer sup­port in sup­port groups, indi­vid­ual and fam­i­ly counseling.

AgeOp­tions

Fund­ing (in whole/​part) for Home Deliv­ered Meals, SHAP, SHIP, Care­giv­er Sup­port, Advo­ca­cy and Out­reach, and Infor­ma­tion and Assis­tance pro­grams are pro­vid­ed through an award/​contract from AgeOp­tions, the Area Agency on Aging of sub­ur­ban Cook Coun­ty, under Title III of the Old­er Amer­i­cans Act as admin­is­tered through the Illi­nois Depart­ment on Aging and Fed­er­al Admin­is­tra­tion on Aging.
