Older Adult Services

Kenneth Young Center provides comprehensive in-home assessments and services to older adults in Elk Grove and Schaumburg townships. We also provide information, assistance, and referrals to adults with disabilities.

We help older adults and caregivers determine their specific needs and the services available to meet them. Our goal is to connect older adults to services that allow them to continue to live as independently as possible.

Support Groups and Educational Programs

Kenneth Young Center’s Older Adult Services Department is committed to providing ample resources to help older adults, caretakers, families, and loved ones through difficult and confusing transitions.

Our Caregiver Resource Center provides information, education, and resources to the family caregivers. A family caregiver encounters a variety of issues including health conditions, changes in behavior, and relationship issues, in addition to the caregiver’s emotional response to these challenges. The caregiver program offers a wide array of services and resources to empower the caregiver in coping with their specific situation.

Educational programs help families understand and learn coping skills to manage conditions like Alzheimer’s, dementia, Parkinson’s, and more.

The process starts with a caregiver assessment and includes individual counselling, educational programs, and a caregiver support group. A plan of care indicates the needs of the family and encourages planning for the future. The Caregiver Specialist supports the family every step of the way as the situation changes and offers ways to promote self-care.

Grandparents Raising Grandchildren Support Group

Grandparents raising their grandchildren have a unique need for support. In this group, grandparents give and get support from other grandparents, share ideas and experiences about raising grandchildren, and get information about resources available to them.

This support group meets the first Monday of every month from 10:00 a.m. to 11:30 a.m. at Kenneth Young Center’s Elk Grove Village office.

Caregiver Support Group

Caring for a family member who needs assistance due to changes in their health or other life-changing events can be overwhelming for the families. Coping with relationship issues and conditions like memory impairment may become especially challenging for the families. In this caregiver support group, you will meet other caregivers, exchange ideas, learn about available resources, receive support, and feel empowered.

This support group meets the third Wednesday of every month from 2:30 p.m. to 4:00 p.m. at Kenneth Young Center’s Elk Grove Village office.

Other educational programs are also scheduled in the community regularly. Information / Assistance

Seniors or caregivers call for help, and we provide information and connections to community services. If an in-home assessment is needed, our intake process is initiated.

In-Home Assessment

A care coordinator visits the home of the senior to conduct a comprehensive assessment of needs.

Care Coordination

With the input of the senior a care plan is developed. The case manager continues to monitor as needs change. Care coordinators coordinate community based services.

For seniors who meet income and disability guidelines, case managers can engage the Illinois Department on Aging Community Care Program (CCP), which includes homemaker services for personal care, shopping, meal preparation and household assistance. These intensive in-home services are meant to prevent premature nursing home placement. CCP also provides adult care, which offers a safe, structured rehabilitative environment and provides caregiver respite.

Advocacy and Support

Assistance in providing eligibility screening and applications for public care benefits.

Special Services

Choices

When frail elders and their families are considering nursing home placement, KYC staff screens applicants for eligibility and assesses for community based services, nursing home placement, or other long term options. KYC staff also completes screens in area hospitals.

Home Delivered Meals

Hot noontime meals are delivered Monday through Friday to those who are unable to manage preparing their own and are homebound.

Money Management

Assistance with check writing, account balancing, budgeting and other financial tasks

Elder Abuse Response

Response, assessment, intervention following reports of elder abuse, neglect or financial exploitation

Senior Health Assistance Program (SHAP)

Provides assistance with applying for Illinois Cares Rx, Medicare D, and Medicate Savings Programs

Senior Health Insurance Program (SHIP)

Offers free health insurance counseling for Medicare beneficiaries and their caregivers

Caregiver Resources

Education on resources and skills for the care of an elderly family member or friend. Tips for self-care and respite, peer support in support groups, individual and family counseling.
