Fer­nan­da’s Story

As Fer­nan­da was slow­ing down after an injury, she found it hard­er to take care of her­self and her home. After dis­cussing some options with her doc­tor, they pro­vid­ed her with KYC’s infor­ma­tion. After con­nect­ing with KYC’s old­er adult ser­vices, Fer­nan­da was able to receive in-home care like a clean­ing ser­vice to sup­port the main­te­nance of her home. She also receives home deliv­ered meals to sup­port her aging safe­ly in place. Fernanda’s care coor­di­na­tor con­nect­ed her with one of KYC’s SHIP vol­un­teers to ensure she was receiv­ing the most out of her Medicare benefits.

Jon­ah’s Story

Jonah Blog

Jon­ah fre­quent­ly spent time in in-patient care because they strug­gled with bipo­lar dis­or­der. Jon­ah was recent­ly tran­si­tion­ing from an in-patient facil­i­ty to per­ma­nent sup­port­ive hous­ing with the help of KYC’s care team. Jon­ah was able to receive the skills to devel­op inde­pen­dent liv­ing while being sup­port­ed in their men­tal health jour­ney. They also learned about addi­tion­al ser­vices like the Drop-In Cen­ter and Inten­sive Recov­ery Group Ser­vices to strength­en the skills they have to suc­ceed and feel confident.

Pop­py’s Story

Poppy Blog

Pop­py was an out­go­ing stu­dent and in many clubs and sports. She often enjoyed many extracur­ric­u­lar activ­i­ties and spend­ing time with her fam­i­ly and friends. Recent­ly, her par­ents noticed she had become with­drawn. Despite usu­al­ly being engaged and active, it became dif­fi­cult for her since she was strug­gling with depres­sion and social anx­i­ety. Pop­py’s par­ents reached out to KYC, where they were able to be con­nect­ed with Child and Ado­les­cent ther­a­py and Behav­ioral Sup­port Men­tor­ing to build her con­fi­dence in the skills and tools she learned in therapy. 

There are many rea­sons why we walk. Whether it is to sup­port our youth pre­ven­tion pro­gram­ming, pro­vide seniors nutri­tious Home Deliv­ered Meals, or ensur­ing our com­mu­ni­ty mem­bers have access to the care they need, when they need it most — your care makes a tremen­dous impact and pro­vides inno­v­a­tive solu­tions for the health of our community!

The Hus­tle For Health 5K will take place on Sat­ur­day, June 22nd at Ken­neth Young Cen­ter’s Elk Grove Vil­lage office where our par­tic­i­pants will gath­er for a fam­i­ly-friend­ly com­mu­ni­ty event.
