Fernanda’s Story
As Fernanda was slowing down after an injury, she found it harder to take care of herself and her home. After discussing some options with her doctor, they provided her with KYC’s information. After connecting with KYC’s older adult services, Fernanda was able to receive in-home care like a cleaning service to support the maintenance of her home. She also receives home delivered meals to support her aging safely in place. Fernanda’s care coordinator connected her with one of KYC’s SHIP volunteers to ensure she was receiving the most out of her Medicare benefits.
Jonah’s Story
Jonah frequently spent time in in-patient care because they struggled with bipolar disorder. Jonah was recently transitioning from an in-patient facility to permanent supportive housing with the help of KYC’s care team. Jonah was able to receive the skills to develop independent living while being supported in their mental health journey. They also learned about additional services like the Drop-In Center and Intensive Recovery Group Services to strengthen the skills they have to succeed and feel confident.
Poppy’s Story
Poppy was an outgoing student and in many clubs and sports. She often enjoyed many extracurricular activities and spending time with her family and friends. Recently, her parents noticed she had become withdrawn. Despite usually being engaged and active, it became difficult for her since she was struggling with depression and social anxiety. Poppy’s parents reached out to KYC, where they were able to be connected with Child and Adolescent therapy and Behavioral Support Mentoring to build her confidence in the skills and tools she learned in therapy.
There are many reasons why we walk. Whether it is to support our youth prevention programming, provide seniors nutritious Home Delivered Meals, or ensuring our community members have access to the care they need, when they need it most — your care makes a tremendous impact and provides innovative solutions for the health of our community!

The Hustle For Health 5K will take place on Saturday, June 22nd at Kenneth Young Center’s Elk Grove Village office where our participants will gather for a family-friendly community event.
